Obituary's Biography:

David Andrew Formica, age 52, a resident of Grayslake, passed away Wednesday, June 7, 2017 at JourneyCare in Barrington.  He was born January 5, 1965 in Downingtown, PA.  He was a member of St. Gilbert Catholic Church in Grayslake.  David was a dedicated firefighter/paramedic at the Grayslake Fire Department for over 18 years.  He was a loving and devoted father who never missed any of his kids’ sporting events and activities.  Dave loved kids and sports and was heavily involved in the Grayslake Youth Baseball Association where he volunteered as coach and president of a league.  He spent his free time tending baseball fields in Grayslake and volunteered many years of his time to the Mundelein High School baseball program. He is survived by his children, Andrew and Heather; his father, Nicholas; his siblings, Nicholas, Michael (Priscilla), Beth, Gregory (Karyn), Amy, and Matthew (Jessica);  nieces and nephews, David, Stephanie, Lauren, Nick, Ben, Jennifer, Tommy, Alexa, Gregory, Nicole, Vance, Maddie and Ryan; and his beloved friend and former wife, Shannon. David was preceded in death by his mother, Doris.  Friends of the family may visit from 4:00 p.m. until 8:00 p.m. on Friday, June 9, 2017 at Strang Funeral Chapel & Crematorium, 410 E. Belvidere Road, Grayslake, IL 60030.  Funeral mass is scheduled for Saturday, June 10, 2017 at 10:00 a.m. at St. Gilbert Catholic Church, 301 E.   2. So sorry for your loss. Dave was a great guy….I remember him with a smile on his face all the time. Prayers being sent for the family and especoially his children.

  3. Dear Andrew: Joshua, Brittany, Shari and myself were very saddened to hear about the loss of your wonderful dad. We send our condolences. Your dad was a great man with a wonderful heart and soul. If you ever need anything from Joshua or myself, please feel free to contact either one of us.
